When you come for your first appointment, please bring your current glasses or contact lenses and any medications you are taking. You may want to arrive 20 minutes before your scheduled appointment to complete initial New Patient Paperwork or you may download, print and complete the four forms below and bring to your appointment.

Emergency vision services are available during office hours by calling the office nearest you. After hours, call 735-2020 or toll-free 1-888-820-7878, and the answering service will arrange contact with a physician on call.

Mid Florida Eye Center accepts Medicare and most health insurance plans. If your insurance company is not listed, please contact the number below as we add new plans regularly.

Our Billing Department will assist you with any necessary paperwork and other requirements relative to your insurance provider. Please call us at 352.735.2020 for more information.
